ABHA health ID not created

Ask the health centre to create your ABHA health ID, and if the centre says it cannot, ask the district health office for a centre that can.

Who is responsible

An ABHA health ID is created at a health facility against the Aadhaar of the person, and once created it carries the vaccination and consultation records across facilities. Where a health centre is not yet able to create one, the district health office can direct you to a centre that is, and creating it once avoids repeating documents at every visit.

Important condition: The health ID is voluntary and is created against a verified Aadhaar; a centre may ask for mobile number linking as well.
