- Doctor absent or patient not attendedThe doctor is absent, on an indefinite leave, or turned the patient away without treatment; the medical officer in charge is accountable.
- Medicine not available or expired in hospitalThe prescribed medicine is not stocked, or what was given is expired or short-dated; the hospital drug store and the medical officer are accountable.
- Ambulance did not come or came lateAn ambulance was called but did not arrive, or arrived after a long delay; the emergency number and the district hospital control room handle this.
- Ayushman card not working or treatment refusedThe Ayushman card is not accepted at the hospital, the family is not found on the list, or cash is demanded despite the card.
- X-ray, ultrasound or test not availableA prescribed diagnostic test is not done in the government hospital and the patient is sent outside at private cost.
- Free medicine not given under government schemeUnder a national health scheme the medicine is meant to be free in a government hospital but is not given, or the patient is asked to buy it outside.
- Pregnancy, delivery or child health service not givenAn antenatal check, institutional delivery benefit, vaccination or child check-up was not provided at the health centre.
- Blood not available in the blood bankA patient needs blood urgently and the blood bank says it is unavailable; the blood bank and the district health office are accountable.
- ABHA health ID not createdYou want a digital health ID (ABHA) and the health centre has not been able to create it, or the number is not working at the hospital.
- Complaint against a private hospitalOvercharging, lack of facilities or negligence by a private hospital; such a complaint goes to the state clinical establishments authority, not the health department.
- Hospital refused to admit the patientA hospital refused admission, including in an emergency, or referred the patient away without a reason; refusing a patient is a punishable offence under the health department rules.
