Under a national health scheme the medicine is meant to be free in a government hospital but is not given, or the patient is asked to buy it outside.

Who is responsible
Medicines under the national health schemes are provided free in empanelled facilities, and a demand for money is a deviation that can be complained about. The written demand, or its absence, together with the scheme name, is what makes the complaint specific and lets the state health agency examine the facility's indents and stock.
Important condition: A patient admitted in a private ward is generally not covered for free medicines, so the ward category matters.
